ING Medical Card (For Cashless Option Only)</p>
<p>With ING Medical Card it guarantees you hospital admission at all panel hospital.² All authorized hospital bills will be settled automatically upon discharge.³<br />
If you have to be admitted to a non-panel hospital, please call our 24 hours service hotline for hassle-free admission with no deposit required.<br />
¹ You may either opt for the cashless option, which comes with the ING Medical Card, or the non-cashless option, where you pay first and submit your claim to ING Insurance for reimbursement later. For premiums of each option please refer to the attached tables on page 9.<br />
² If you are hospitalized during the first three (3) months of your policy, you will be required to pay first and then submit your claim to ING Insurance for reimbursement later, subject to the terms &amp; conditions of the policy.<br />
³ You are required to pay a Self-Insured Deductible amounting to RM50 for the hospitalization and surgical expenses which you have incurred.</p>
<p>Referral Emergency Assistance Program<br />
You get to enjoy extensive domestic and international assistance programs with your ING MediPlus. Just call our 24 hour service hotline: 603-7665 2550 or 603-7954 4717 from any part of the world and reverse the call charges.</p>
<p>The follow are the services offered:<br />
International Medical Assistance Program-emergency medical evacuation and repatriation, repatriation of mortal remains, medical assistance, travel information.<br />
Domestic Medical Assistance Program-emergency medical evacuation and repatriation, medical assistance.<br />
Car Assistance Program*-emergency towing and minor roadside repair, car rental assistance, referral to service centres.<br />
Home Assistance Program*-plumbing, locksmith, general repair, air conditioning and pest control assistance.<br />
Travel Assistance Program*-provides information concerning visa, passport and inoculation requirement; assists in location of lost items, emergency message relay, arrangements of flights, legal referral, referral to interpreter/translator and weather and foreign exchange information.<br />
Services marked with an asterisk* are referral services only. The cost of the actual services shall be borne by the policyholder. These services are provided by a third party and the continuation of these services are subject to the agreement between ING Insurance Berhad and Asia Assistance Network (M) Sdn. Bhd.<br />

Choice of Plan<br />
ING MediPlus offers you a choice of three attractive plans designed to suit your budget and healthcare needs.<br />
Policy Renewability<br />
Your ING MediPlus insurance policy is renewable up to age 85 whilst the Company continues to transact hospitalization and surgical business. This policy renewability is subject to the aggregate lifetime claim limit of 3 times the Overall Annual Limit and insurability of the occupation class.</p>
<p><strong>120 Days Exclusion for Specific Illness</strong></p>
<p><strong> </strong></p>
<p><strong>ING MediPlus imposes 120 days exclusion on the following conditions:<br />
Hypertension, diabetes mellitus and cardiovascular disease<br />
Hernias, haemorrhoids, fistulae, hydrocele, varicocele<br />
All tumours, cancers, cysts, nodules, polyps, stones of the urinary system and biliary  system<br />
Endometriosis including disease of the Reproduction system<br />
All ear, nose (including sinuses) and throat conditions<br />
Verterbo-spinal disorders (including disc) and knee conditions</strong></p>
<p><strong> Exclusions/Limitations of Coverage</strong><br />
No benefits shall be payable for hospitalization, surgery or charges caused directly or indirectly, wholly or partly, by any one of the following:<br />
Pre-Existing conditions.<br />
Any medical or physical conditions arising within the first thirty (30) days of the Insured Person’s cover or Date of Reinstatement whichever is latest except for accidental injuries.<br />
Plastic/Cosmetic surgery, circumcision, eye examination, glasses and refraction or surgical correction of nearsightedness (Radial Keratotomy or Lasik) and the use or acquisition of external prosthetic appliances or devices such as artificial limbs, hearing aids, implanted pacemakers and prescriptions thereof.<br />
Dental conditions including dental treatment or oral surgery except as necessitated by Accidental Injuries to sound natural teeth occurring wholly during the Period of Insurance.<br />
Private nursing, rest cures or sanitaria care, illegal drugs, intoxication, sterilization, venereal disease and its sequelae, AIDS (Acquired Immune Deficiency Syndrome) or ARC (AIDS Related Complex) and HIV related diseases and any communicable diseases required quarantine by law.<br />
Any treatment or surgical operation for congenital abnormalities or deformities including any hereditary conditions.<br />
Pregnancy, child birth (including surgical delivery), miscarriage, abortion and prenatal or postnatal care and surgical, mechanical or chemical contraceptive methods of birth control or treatment pertaining ton infertility. Erectile dysfunction and tests or treatment related to impotence or sterilization.<br />
Hospitalization primarily for investigatory purposes, diagnosis, X-Ray examination, general physical or medical examinations, not incidental to treatment or diagnosis of a covered Disability or any treatment which is not Medically Necessary and any preventive treatments, preventive medicines or examinations carried out by a Physician, vitamins/food supplements and treatments specifically for weight reduction or again.<br />
Any Outpatient treatment unless specifically provided under this policy.<br />
Suicide, attempted suicide or intentionally self-inflected injury while sane or insane.<br />
War or any act of war, declared or undeclared, criminal or terrorist activities, active duty in any armed forces, direct participation in strikes, riots and civil commotion or insurrection.<br />
lonizing radiation or contamination by radioactivity from any nuclear fuel or nuclear waste from process of nuclear fission or from any nuclear weapons material.<br />
Expenses incurred for donation of any body organ by the Life Insured and costs of acquisition of the organ including all cost incurred by the donor during organ transplant and its complications.<br />
Investigation and treatment of sleep and snoring disorders, hormone replacement therapy and alternative therapy such as treatment, medical service or supplies other than Western medicines, including but not limited to chiropractic services, acupuncture, acupressure, reflexology, bone setting, herbalist treatment, message or aroma therapy or other alternative treatment.<br />
Care or treatment for which payment is not required or to the extent which is payable by any other insurance or indemnity covering the Life Insured and Disabilities arising out of duties of employment or profession that is covered under a Workman’s Compensation Insurance Contract.<br />
Psychotic, metal or nervous disorders, (including any neuroses and their physiological or psychosomatic manifestations).<br />
Costs/expenses of services of a non-medical nature, such as television, telephones, telex services, radios or similar facilities, admission kit/pack, medical report and other ineligible non-medical items.<br />
Sickness or injury arising from racing of any kind (expect foot racing), hazardous sport such as but not limited to skydiving, water skiing, underwater activities requiring breathing apparatus, winter sports, professional sports and illegal activities.<br />
Private flying other than as a fare-paying passenger in any commercial scheduled airlines to carry passengers over established routes.<br />
Expanses incurred for sex changes.<br />
Any treatment following an unlawful act.</p>
<p>Please note that the benefits offered under Referral Emergency Assistance Programme are subject to exclusions. Please refer to the policy documents for details.<br />
Questions &amp; Answers<br />
<strong><em> Q: Who is eligible?</em></strong><br />
A: Anyone between the ages of 30 days to 69 years old can apply for ING MediPlus. Acceptance however is subject to underwriting. foreign citizens who apply to participate in MM2H programme</p>
<p><strong><em> Q: Is this policy eligible for tax relief?<br />
</em></strong> A: Yes. When you purchase this Plan, you are entitled to tax relief subject to the provisions of the Income Tax Act 1967. Your premiums are eligible for tax deduction up to a maximum of RM3,000 per year (inclusive of insurance premium for Education Plans). The amount of tax relief may vary as declared by the Malaysian Inland Revenue Board.<br />
<strong><em> Q: When I receive the Plan and I do not agree with any of the terms and conditions, can I still reconsider?<br />
</em></strong> A: Yes, you can. Within fifteen (15) days from the date the policy is delivered to you, you are free to review the policy and cancel the policy without assigning any reason. ING will refund all the premiums already paid less any expenses incurred for medical examination (if any). We want you to understand the full benefits of this Plan for you and your family to ensure that the Plan fulfils your needs.<br />
<strong><em> Q: How do I make a claim?</em></strong><br />
A: Should you need to make a claim, please contact your ING agent 012 233 8369 or Business Manager</p>
<p>Please note:<br />
Premium payment shall be made on a yearly basis until the policy anniversary nearest to 70th birthday of life insured.<br />
Immediate written notice must be given to ING of any change to your occupation. The Company shall be entitled to determine if any additional premium is payable or to terminate the policy if your occupation has changed to one which is not insurable under this plan.<br />
Upon renewal, premium may increase based on your Attained Age. The Attained Age of a Life Insured at a particular Policy Anniversary Date is computed by adding the Age of Entry of the Life Insured to the number of completed Policy Years from the Policy Date.<br />
Premiums are not guaranteed and ING reserves the right to revise the premiums upon renewal of policies by way of not less than 30 days’ notice in writing. Factors contributing to the increase in premiums are age-specific and plan-specific claims experience, medical inflation and corresponding increase in Overall Annual Limit.<br />
If you have received the compensation or reimbursement of the medical expenses incurred from any other medical insurance, any employee benefit or any Government law and program, the benefit payable shall be limited to those medical expense which are not fully reimbursed under such scheme.<br />
No benefit whatsoever shall be payable for any medical treatment outside Malaysia apart from Singapore and Brunei if the Life Insured resides or travels outside Malaysia for more than ninety (90) consecutive days.<br />
If the Life Insured seeks treatment overseas, benefits in respect of the treatment shall be covered subject to the exclusions and conditions specified in this plan. Overseas treatment (apart from Singapore and Brunei) of a disease, sickness or injury where there is no Medical Emergency or for chronic conditions where treatment can reasonably be postponed until return to Malaysia are excluded.<br />
For medical treatments outside Malaysia, Singapore and Brunei, the medical expenses in respect of such treatment shall be payable only for the emergency treatment while traveling overseas for not more than ninety (90) consecutive days. The benefit payable for any medical treatments outside Malaysia shall be limited to the Reasonable and Customary Charges for such equivalent treatment in Malaysia.<br />
You should ensure that the plan meets your needs and that the premium payable under the policy is an amount that you can afford. The policy will be terminated if premiums are not paid within the 30 days grace period from the due date.<br />

<p>Newly launched ING Public Takaful Ehsan Bhd aims to achieve RM31 million in premiums or contribution in its first year of operation, driven by potentials in the Takaful market which still has low penetration rate in Malaysia.</p>
<p>&#8220;Moving forward, we target to grow our market share to 10 per cent on the back of projected contributions of over RM800 million by 2015,&#8221; said its Chief Executive Officer Saiful Yazan Ahmad after the launch of the joint venture company here.</p>
<p>The company is a strategic alliance between ING Management Holdings (Malaysia) Sdn Bhd with 60 per cent equity participation, and both Public Bank Bhd and Public Islamic Bank Bhd with 20 per cent stake respectively.</p>
<p>&#8220;We will leverage on our shareholders&#8217; strength to have at least 10,000 agency forces and also distribute our products through Public Bank and Public Islamic Bank&#8217;s network of 250 branches,&#8221; Saiful said.</p>
<p>The local Takaful business still represents a huge untapped market for the company, he said.</p>
<p>&#8220;The penetration rate of Takaful among the 16 million Muslims in the country is only around 10 per cent, compared to the conventional insurance penetration which is around 45 per cent,&#8221; he said.</p>
<p>A few more products is slated for introduction this year, namely the investment link family Takaful, term personal assurance and medical coverage products.</p>
<p>Saiful said the company may also venture into foreign markets, but added there was nothing concrete yet.</p>
<p>Public Bank&#8217;s Managing Director and Chief Executive, Tan Sri Tay Ah Lek said the initial revenue contribution to Public Bank may be low but expected the contribution to be significant in three years.</p>
<p>Also present at the launch, Bank Negara Malaysia Deputy Governor Datuk Mohd Razif Abd Kadir said the takaful industry was poised to benefit in the years ahead, given the large untapped market with only 54 per cent of the population having a life insurance or family takaful policy. &#8212; BERNAMA</p>

<p>Contractor’s All Risk Insurance provides comprehensive coverage to all works to be executed in accordance with the contractor’s contract, temporary works, material, construction plant and equipment brought into the site and liabilities arising out of the performance of the contract. The policy covers against accidental and unforeseen physical loss or damage to the property insured occurring during the period of insurance.</p>
<p>Under the material damage section of the policy, any sudden and unforeseen physical loss or damage caused by fire, lightning, explosion, flood, inundation, windstorm of any kind, earthquake, theft, burglary, bad workmanship, negligence, lack of skill, lack of experience, malicious acts or human error, or any other sudden and unforeseen event such as loss or damage due to collapse, foreign objects and on-site transport of items to be constructed are covered.</p>
<p>The liability section of the policy provides indemnity in respect of third party liability which the insured shall become legally liable to pay as damages consequent upon :</p>

<p>Specially designed to meet requirements of compensation as spelt out in the Workmen&#8217;s Compensation Act 1952. All employers are legally obligated to insure workmen against accidents at work or occupational illness.</p>

<p>Foreign Workers Compensation Scheme (FWCS) was created to protect the interest and welfare of all foreign workers in Malaysia. <p>Boilers &amp; Pressure Vessels Insurance is designed to cover loss or damage as a result of explosion or collapse of boilers and other pressure vessels caused by the force of internal steam or fluid pressure.</p>
<p>Apart from damage to the boilers and pressure vessels, the policy also covers for damage to surrounding property and legal liability of the Insured for any death, bodily injury or damage to third party properties.</p>

<p>Electronic Equipment Insurance is specially designed to cover all electronic equipment such as computers, including external data media, against loss or damage due to sudden and unforeseen causes provided such causes are not specifically excluded in the policy.</p>
